At Faith Hospice, our mission calls us to provide compassionate care in Christ's name, and we are grateful for the generous community of friends who help make our commitment a reality. Faith Hospice provides this care by addressing the needs of the whole person—physical, emotional, and spiritual—as well as providing support to family members, loved ones, and caregivers. Care is provided in private homes, assisted living and skilled nursing facilities, and hospitals and at Trillium Woods, our hospice inpatient facility.

HOW YOU CAN HELP 

There are many ways you can help those we care for through Faith Hospice. Whether it's volunteering your time, attending one of our fundraising events, or making a donation, your support will help us deliver on our promise of care.

Gifts to the Faith Hospice Circle of Faith support an endowment fund that will help people now and in the future receive hospice care services from Faith Hospice.

MEMORIAL TREES OF LIFE

To honor the memory of a loved one, we offer an opportunity through our Memorial Tree of Life. Handcrafted by local artist Toni Lee, this permanent display is located at Trillium Woods. Memorial leaves, engraved with the name of your choice, are available at three different levels and offer a beautiful and lasting tribute to loved ones. To purchase a leaf in honor of your loved one online, please indicate: leaf level, amount of gift, and name of loved one in the comments section of the donation form.

1892 HERITAGE SOCIETY

The 1892 Heritage Society honors donors who provide a planned gift for Faith Hospice, Holland Home, or the Holland Home Foundation. Planned gifts include a gift through a will or trust; naming one of our organizations as a beneficiary of an IRA, life insurance policy, or retirement plan; a charitable gift annuity; or gifting a portion or all of the refundable amount from your Continuing Care Agreement.
