Planning ahead ensures that your values and wishes are honored while offering peace of mind to your loved ones. At Faith Hospice, we believe everyone should be empowered to make healthcare decisions that align with their beliefs, goals, and quality-of-life priorities—no matter what the future holds.

  • Reduce uncertainty and stress for loved ones

  • Give individuals a voice in their own care
  • Hospice Care: Supportive care for those with six months or less to live who choose to stop curative treatment. The focus is on comfort, dignity, and quality of life for both the patient and their loved ones.
  • Palliative Care: Specialized care that focuses on symptom relief and improving quality of life for individuals facing serious illness, regardless of life expectancy.
  • Advance Directives:  Legal documents such as a living will or durable power of attorney for healthcare that outline your medical preferences and designate a trusted decision-maker.
  • Additional preferences to include:
    • Do Not Resuscitate (DNR) orders
    • Pain management and comfort measures
    • Where you wish to receive care (such as at home or in an inpatient hospice setting)

  5. Share Your Plan: Provide copies of your advance care plan to your family and healthcare providers so they can follow your wishes if needed.
