Pilot Program

Faith Hospice is proud to be applying for a pilot program through Primary Care First that would allow for an even further extension of care for our seriously ill patients.

The program seeks to identify Medicare beneficiaries who are high need, seriously ill, and have experienced a fragmented pattern of care and successfully transition them to primary care practices. We are looking to develop a network of primary care physicians who are willing to accept referrals from us to manage the needs of these patients.
