Faith Hospice

What is hospice?

Hospice eligibility

To be hospice eligible, the patient’s physician must certify that life expectancy, if the illness runs its normal course, is expected to be six months or less. When the patient is ready to stop seeking curative treatment, Faith Hospice is there to help.

A hospice patient can receive care for two 90-day periods, which can be followed by an unlimited number of 60-day periods as long as the hospice team can certify that the illness continues to be terminal and the patient likely has less than six months to live.

Hospice care is for any person who has a life-limiting illness. Patients with both cancer and non-cancer illnesses are eligible and include:

  • Heart disease
  • Pulmonary disease
  • Neurological disease (such as Parkinson’s or ALS)
  • Liver disease
  • Renal disease
  • HIV/AIDS
  • Alzheimer’s disease or dementia
