The hospice care team
Hospice care is provided in conjunction with the patient's primary care physician by an inter-disciplinary group guided by our medical directors. Some of the professionals who may be involved in patient care include:
Personal physician
The patient's personal physician works with the patient, family, and our interdisciplinary team to create a plan of care.
Faith Hospice medical directors
Faith Hospice's medical directors are skilled in hospice and palliative care medicine.
Registered nurse case managers
Our case managers are registered nurses (RNs) who provide primary care for the patient and coordinate care with the other members of the care group. Care is available 24 hours per day, seven days per week, either through home visits from a member of the care group or at the Faith Hospice residence Trillium Woods.
Licensed practical nurses (LPNs) and hospice aides (CNAs)
Our experienced LPNs and CNAs assist with the patient's personal needs as well as help to promote comfort and symptom control.
Social workers
Our social workers (LMSWs) provide emotional support for the patient and their family and can help assist with issues surrounding finances, funeral arrangements, etc.
Spiritual staff
Our Faith Hospice chaplains are available to all patients and their families although your own clergy person is encouraged to remain involved. A chapel is available at our Faith Hospice residence Trillium Woods.
Volunteers
Our trained volunteers offer companionship, support and assistance to patients and their families. They may run errands, do light housekeeping or provide respite for caregivers.
Bereavement counselors
Our bereavement counselors provide individual or group support to families before and after the death of their loved one.
Therapists
Based on the patient's plan of care, physical, occupational and speech therapy is available.
