Giving to Faith Hospice

The Faith Hospice Mission is to provide hospice care “wherever you put your head down to sleep at night”. Faith Hospice provides this care by addressing the needs of the whole person — physical, emotional and spiritual — as well as providing support to family members, loved ones and caregivers in private homes, assisted living, skilled nursing facilities, hospitals, and at Trillium Woods. We are able to carry out the Mission through your generous donations to the Patient Assistance Fund, this allows us to offer care to patients and families that are unable to afford hospice care.

The Faith Hospice Information and Giving Guide tells more about all that Faith Hospice does and how you can help.

Special note regarding memorial gifts

Many families identify Faith Hospice as a designated charity for memorial gifts. If you’d like to honor a memorial invitation, please identify, on the face of your check, in a short note or on the memorial envelope, the person in whose honor your gift is made. Faith Hospice will inform a designated family member or loved one of your gift (but not the amount of your gift).

Faith Hospice is a non-profit organization. Gifts are tax deductible as donations.

A note or short letter describing the purpose of your gift will help us honor your gift intentions. Gifts not otherwise designated will be directed to the Faith Hospice Capital Campaign.
