RN Hospice Case Manager

Responsibilities

Coordinate the personal care and comfort of assigned patients using the team approach. Assess and develop plan of care (POC), and evaluate effectiveness of the POC. Duties include administering medications, performing treatments as prescribed by the physician or as warranted by the patient’s condition, and providing instruction to patients, families, and their caregivers.

Skills required

Requires licensure as an RN in the State of Michigan. Requires a minimum of 1 year of acute care, skilled nursing or hospice nursing experience. Must have reliable means of transportation, valid drivers license, and proof of auto insurance.

Other information

This position is full-time with a Monday through Friday schedule. There is an on-call rotation as well as 2 Holidays per year rotation. This includes full-time benefits, paid time off, and retirement savings plan.

RN Hospice Staff Nurse - per diem

Responsibilities

Assist during high census periods, covering for full-time team members who are on vacation or leave, and assisting with special projects for hospice. Assistance includes but is not limited to, making visits to hospice patients in the community, coordinate the personal care and comfort using the team approach. Assess and develop plan of care (POC) and evaluate the effectiveness of the POC. Administer meds, perform treatments as prescribed by the physician or as warranted by the patient's condition, and provide instruction to patients, families, and their caregivers.

Skills required

Requires RN licensure in the State of Michigan. Prior hospice is experience is highly preferred as is experience working with the elderly. Employee must have reliable means of transportation to make visits and proof of valid Michigan driver's license and vehicle liability insurance.

Other information

This position is per diem, working at least once a month and then filling in as needed to assist the hospice team. Hours are not guaranteed.

Center Manager (RN or MSW) - Care Resources

Responsibilities

Manage the daily functions of the adult day health center in conformance with Care Resources policies, procedures, and standards and PACE regulations. Facilitate the PACE interdisciplinary team.

Skills required

Requires licensed RN or MSW in the State of Michigan; requires a minimum of 3 years of experience in a health care setting and strongly value experience in health plan administration. Requires at least one year of supervisory related responsibilities. All Care Resources staff must have at least one year of experience within their discipline of working with the frail or elderly population.

This position is full-time and serves as a key member of the management team for Care Resources. It includes full-time benefits, retirement savings plan, and paid time off.

CNA

Responsibilities

Provide residents with routine daily nursing care in accordance with established procedures. Observe and report data regarding resident care accurately.

Skills required

State certification required. Must be in good standing with the state registry.

Helping Hands Assistant

Responsibilities

Assists on a non-clinical basis in the PACE center as well as some home care possible. Helps transport within the center, pass meals, assist with activities. Provides support for other staff members.

Skills required

Must have a minimum of 1 year geriatric interactment. Must have own transportation and valid drivers license/current auto insurance. Must be reliable and a self motivator.

This position is at Fulton. It is a part-time position during 1st shift, and provides 20 hours per week.

Home Assistant

Responsibilities

Providing in home care for the elderly within the community through personal cares, light housekeeping, basic food prep, and transportation needs. Also includes assisting with ambulation and activities of daily living.

Skills required

Valid MI drivers license, reliable source of transportation, and current auto insurance. Experience working with the elderly is preferred but not required. Must have strong interpersonal skills and be organized and self motivated. Successful candidates have good computer skills and have access to email.

This position is at Helpers of Holland Home. It is a per diem position during all shifts shift, and provides a varying amount of hours per week.

Housekeeping

Responsibilities

Cleaning of resident rooms, offices, and common areas. Duties include, but are not limited to, dusting, cleaning, disinfecting, vacuuming, etc.

Skills required

1-2 years of housekeeping experience required. Interpersonal skills necessary to effectively communicate with all levels of staff and residents in the exchange of information. Ability to read, speak, and understand the English language. Ability to reach, stoop, bend, kneel, and/or crouch while performing required duties.

Some evening, weekend, and/or holiday hours will be required.

LPN Response Nurse - assisted living

Responsibilities

Provide nursing care for residents. Duties include administering medication, resident assessments, changing resident dressings, performing catheterization, taking and recording resident’s vital signs, performing treatments as prescribed by physician or as warranted by resident’s condition, etc. Provides guidance and supervision to Resident Assistants. Promotes person centered care in an environment that recognizes quality of life and emotional health for all residents and co-workers.

Skills required

Requires licensed LPN in the State of Michigan and CPR certification. Requires prior experience working with the geriatric population, preferably in an assisted living or long term care setting.

Details

This position is at Raybrook. It is a part-time position during 3rd shift, and provides 24 hours per week.

Resident Assistant

Responsibilities

Provide geriatric residents assistance with activities of daily living which may include personal cares, ambulation, help with meals, transport within the campus, as well as other responsibilities.

Skills required

This position offers on the job training, although previous experience working with the elderly is valued. Med pass experience is preferred for this position. Must have understanding of aging and dementia. Must be organized and self-motivated

RN Navigator - HHS, Health Options

Responsibilities

Tandem365 is seeking clinical case management staff who have the ability and skills to help our participants navigate through the health care system . This is a community based program, where the RN Navigator will be providing participant support, caregiver support, and assessment of needs in the home, doctor’s office, or a facility.

Skills required

This position requires an RN license in the State of Michigan. The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Must have the ability to work well as a team as well as independently. Minimum of 2 years of acute care and/or certified home care experience required. Prefer someone with prior experience in community based services providing case management.

Social Work Health Care Navigator (MSW) - HHS, Health Options

Responsibilities

Holland Home, Clark Retirement, Life EMS, Porter Hills, and Sunset Retirement Communities are embracing health care reform by joining together to create a new program called Tandem365; which allows older adults to age in place. Tandem365 is seeking clinical case management staff who have the ability and skills to help our participants navigate through the health care system. In this community based program, the MSW Navigator will be providing participant assessment, care, teaching and support, caregiver support, and assessment of needs in the home, doctor’s office, or a facility. The MSW Navigator works as a team with an RN Navigator and interdisciplinary team to develop plan of care, including supportive services necessary for promoting optimum health and decreasing hospitalization and emergency room care. MSW interventions may include education, counseling, referrals, coaching, and therapeutic conversation to the Tandem365 participants in the home setting.

Skills required

The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Should be comfortable caring for some individuals with behavioral health conditions in addition to complex and chronic medical conditions. Must have the ability to work well as a team as well as independently. Must have the ability to work within a multi-cultural community with varied socioeconomic and psychosocial needs. Must have familiarity with community resources for connecting individuals to and working collaboratively with community agencies. Must have the ability to work independently and problem solve in a timely manner, present and participate in daily interdisciplinary team meetings, maintain ability to be flexible to work assignments, be comfortable in meeting participants in their home for enrollment, assessments, and routine visits.
